null The manufacturing process of fitting the child proof cap to the bottle was not 100% efficient in ensuring the child proof mechanism became engaged resulting in a number of caps which could simply be twisted off. This situation presents a risk of a child opening the bottle top and drinking the contents, causing burns to mouth and internal organs.
